Description

Alcohol and Other Drug (AOD) Support Connections, provides assertive outreach and case management to people engaging in AOD use. The program provides up to three months of case management support to individuals aged 16 years and over who are experiencing issues related to AOD. Case management support is delivered via outreach with case workers able to meet with clients at agreed locations within the community. The program uses a strengths-based approach that incorporates assessment, case planning and coordination, advocacy, referral, harm reduction information and relapse prevention strategies. The program addresses client goals around AOD use while increasing clients safety, health and well-being.

No exclusions, i.e. the a low threshold service able to see people whether they have used drugs or not, people can attend whilst on medication or going through withdrawal or while Opioid Management Therapy
